              "text": "Be sure to bring these items with you to your consultation appointment: \n\n* A referral slip, if applicable \n* X-rays taken by your general dentist (your general dentist may send these to our office ahead of time)\n* A list of current medications\n* Dental and medical insurance information \n* A valid ID (e.g., driver’s license)\n\nYou can complete and submit new patient paperwork online to save time on the day of your appointment. If you do not complete the forms online, we ask that you arrive at your appointment a few minutes early to complete the necessary paperwork. \n\n#### Patients Under 18 \n\nPatients under the age of 18 must be accompanied by a parent or legal guardian at the consultation appointment."
